The purpose of our short Get-to-know-you Survey is to make your First Visit at MAT as enjoyable and successful as possible. Your survey responses will help your MAT teacher understand how you learn best so that they can adjust their teaching style to your unique personality. 

Please complete this survey before coming to your First Lesson/Class at CFMA. For younger students: parents, please help with the answers, especially regarding your child’s preferred learning style.
